TDSB Health Pass

Please find a copy of the Health Pass below for your information. The school will be printing and providing for families when the school reopens.

The pass is a self-assessment for students to be completed with parents with the child(ren).

The parents are required to date and sign the pass daily and returned with their child(ren) upon arrival to school when they meet their teachers.
